Original Pre-numbered, Preprinted duly signed and stamped receipt against final bill of the Hospital
This means that the claimant has not submitted some or all of the receipts of payments made to the hospital to settle the hospital bill. Of the submitted payment receipts, some or all of the receipts have not been printed on the letterhead of the hospital. Receipts printed on blank papers, with or without hospital stamp/seal, are not acceptable in reimbursement claim. It is mandatory to submit payment receipts for the total hospital bill amount (even one receipt missing or not in the proper format is not acceptable in a claim).
